Question Dynamically Genreate Controls Using javascript

Hi friends,

I am facing a little problem that is : i want that when user click on add more button on page a new row of controls(one text box,one combo box) should be genrated and controls shold have unique name.

for example i want to take skill set input from a user and on form i give him 5 text box by defaul and one button add more skill to add new text box each time he click on it.

Please help it's so urgent...
